Its not a striking error as this example didn't go through any part of the minting process. Its possible that it was a test piece for use in vending machines, etc that wasn't returned. Correct size and weight (12g). A rare item. NB. Coin blanks, like everything else, are available from China. They look nothing like this one. Examples can be
